Does CBD Improve Gut Health?
CBD is increasingly discussed in connection with “gut health,” the microbiome and digestion.
But current human research does not establish ordinary CBD oil as a way to improve or balance the gut microbiome.
Scientists have legitimate reasons to investigate cannabinoids and gastrointestinal biology. The endocannabinoid system is involved in several processes within the digestive system, and laboratory and animal studies have produced interesting findings involving intestinal inflammation, motility and gut microorganisms.
Human evidence is far more limited.
A 2025 systematic review looking at cannabis, cannabinoids and the human microbiome identified only nine relevant human studies.
Those studies investigated very different populations, cannabinoid exposures and areas of the microbiome, and produced a mixture of positive and negative findings. Read the 2025 systematic review on PubMed.

So describing CBD as something that “balances your gut bacteria” currently goes beyond the evidence.
What Is the Gut Microbiome?
The gut microbiome is the community of microorganisms living in the gastrointestinal tract.
It includes a vast range of bacteria and other microorganisms.
Researchers have linked differences in the microbiome with many aspects of health and disease, but microbiome science is extremely complex.
There is no single universally “perfect” microbiome.
That also means claims that a supplement “balances,” “repairs” or “optimises” gut bacteria need much stronger evidence than simply showing that microbial populations changed during an experiment.
Why Are Cannabinoids Being Studied in Gut Research?
The endocannabinoid system is present throughout the gastrointestinal system and participates in signalling connected with processes including intestinal movement, sensation and immune function.
Researchers have therefore investigated whether cannabinoids might influence intestinal motility, inflammatory signalling, gut permeability and microbial populations.
This makes CBD and gut biology a legitimate research topic.
But once again:
a plausible biological mechanism is not proof of a consumer health benefit.
What Does the Human Microbiome Research Show?
A comprehensive systematic review published in 2025 examined human studies involving cannabis, cannabinoids, cannabinoid-like mediators and the microbiome.
Only nine studies met the review criteria: two clinical trials and seven observational studies.
They looked at different microbiomes — including gastrointestinal, oral and other microbial communities — in very different populations.
The results were not uniform.
Researchers observed both positive and negative associations depending on the clinical context, cannabinoid exposure and microbial measure being examined.
That makes the evidence interesting.
It does not establish:
“CBD creates a healthier microbiome.”
Another important limitation is that the systematic review was not simply nine trials of purified CBD oil.
It included broader cannabis and cannabinoid exposures.
Those findings therefore cannot automatically be applied to CBD isolate.
What Did the Randomized CBD Gut-Microbiome Trial Find?
A more directly relevant randomized trial investigated CBD itself.
Sixteen sedentary adults with overweight or obesity were assigned to either placebo or 30mg CBD every 12 hours for four weeks.
Researchers examined glucose tolerance, inflammation and faecal microbiome characteristics before and after the intervention.
Compared with placebo, CBD did not appreciably alter characteristics of the gut microbiome.
Inflammation was also not appreciably modified.
This was a small pilot study involving people without diabetes, so it does not answer every question about CBD and gut health.
But it is valuable because it demonstrates why claims that CBD has been proven to improve the human microbiome are premature.
Does CBD Increase “Good” Gut Bacteria?
There is currently not enough human evidence to say that ordinary CBD oil reliably increases beneficial gut bacteria.
Microbiome research sometimes identifies changes in particular bacterial groups after cannabinoid exposure.
But individual microorganisms cannot always be neatly divided into universally “good” and “bad” categories.
The effect can depend on the person, surrounding microbial community, diet, health status and many other factors.
This is another reason we should avoid simplistic claims such as:
“CBD feeds good bacteria and removes bad bacteria.”
CBD and Intestinal Motility
Another area of research is intestinal motility — the movement of contents through the digestive system.
A 2023 systematic review examined CBD and intestinal motility.
It found promising laboratory and animal evidence, including research in animal models of colitis.
But the authors also found that clinical trials had not established a conclusive benefit for intestinal motility disorders, and noted significant differences in dose, formulation and study design. Read the intestinal-motility review on PubMed.
So it would be premature to recommend retail CBD for constipation, diarrhoea, IBS or other motility-related conditions on the basis of this research.
Does CBD Reduce Gut Inflammation?
CBD has been studied extensively for effects on inflammatory pathways.
But human evidence does not support presenting ordinary CBD oil as a treatment for gastrointestinal inflammation.

The key distinction is that changes in laboratory inflammatory pathways are not the same thing as showing improved gastrointestinal disease in humans.
What About CBD for IBS?
CBD and other cannabinoids are sometimes discussed in connection with irritable bowel syndrome.
IBS and inflammatory bowel disease are different conditions.
Current clinical evidence has not established retail CBD as an effective IBS treatment.
The intestinal-motility systematic review found interesting preclinical evidence but concluded that human trials have not yet produced a conclusive benefit for intestinal motility disorders.
The old gut article's statement that CBD may benefit IBS should therefore be removed rather than turned into a product recommendation.
What About CBD for IBD or Crohn’s Disease?
Inflammatory bowel disease includes conditions such as Crohn’s disease and ulcerative colitis.
Those disease-specific questions deserve their own clinical evidence review.

Does CBD “Heal the Gut”?
There is currently no adequate human evidence for describing ordinary retail CBD oil as something that “heals the gut.”
“Gut healing” is also a very broad phrase that can refer to numerous different biological processes and medical conditions.
A supplement altering one laboratory pathway does not demonstrate healing of damaged intestinal tissue.
We should therefore avoid that terminology.
Can CBD Improve Digestion?
There is not enough human evidence to say that CBD reliably improves digestion in healthy people.
Digestion includes many processes — stomach emptying, intestinal motility, digestive enzymes, absorption, microbial activity and more.
CBD research has touched some of these areas, but it has not demonstrated a general “better digestion” effect that can be claimed for retail CBD oil.
What About CBD and the Gut-Brain Axis?
The gut and nervous system communicate through multiple pathways, sometimes collectively described as the gut-brain axis.
The microbiome is one component of this complex relationship.
Cannabinoid and endocannabinoid signalling are also being investigated within it.
But this is an emerging field.
It would be an overstatement to tell consumers that CBD improves mood, stress or mental wellbeing by “balancing the gut-brain axis.”
This article should explain the research concept without promising an outcome.
Is Higher-Strength CBD Better for Gut Health?
There is no clinical evidence establishing a 40% retail CBD oil as more beneficial to the microbiome or digestive health than a 10% oil simply because it is more concentrated.
Strength tells you CBD concentration.
It does not tell you that the product will improve gut bacteria or intestinal function.
Is CBD Isolate Better for the Gut?
There is also no good clinical evidence showing that CBD isolate is superior for gut health.
CBD isolate describes the cannabinoid composition of the product.
It should not be marketed as being “gentler on the gut,” “better for digestive sensitivities” or more effective because other hemp compounds have been removed unless appropriate evidence actually establishes those claims.
Does CBD Oil Irritate the Stomach?
People can respond differently to foods, oils and supplements.
A reaction to a particular CBD product could potentially relate to CBD itself, the carrier oil, another ingredient or an individual's circumstances.
We should not claim that full-spectrum CBD causes digestive irritation while isolate avoids it.
Someone experiencing persistent gastrointestinal symptoms should seek appropriate medical advice rather than trying to diagnose the cause from the CBD extract type.
CBD, Medication and Digestive Conditions
Current FSA guidance says CBD can interact with certain medications.
People taking medication should avoid CBD unless under medical direction.
The FSA also advises people with underlying health conditions to avoid consuming CBD unless under medical direction. Read current FSA guidance.

What Does the FSA Say About CBD?
The Food Standards Agency currently advises healthy adults consuming CBD foods not to exceed 10mg CBD per day.
It also states that CBD foods have no authorised health claims, and claims that CBD foods treat illness are not permitted.
People taking medication and those with underlying health conditions are advised to avoid CBD unless under medical direction.
The 10mg figure is consumer-safety guidance for healthy adults.
It is not a dose for improving gut health or modifying the microbiome.
